Frequently Asked Questions

The following information is required to get on the eligibility list:

1. Name of individual 75 years or older 
2. Date of Birth
3. Contact phone number
4. Contact email address (if utilized)

Signing up for the 75 YOA or older Portsmouth Eligibility List is easy!  Either:

• E-mail vaccine@portsmouthri.com with required information OR
• Leave a message at our (401) 643-0323 hotline that is open from 9 a.m. to 4 p.m. Monday through Friday; OR
• Go to our on-line sign-up link and enter information at https://rwu.co1.qualtrics.com/jfe/form/SV_eJrE4ouKOsmDWku.

If you have already sent an e-mail or spoken to a Town Hall staff member and provided your contact information and age, you are already on the list.  No further action is required by you, and you will not receive a return phone call unless more information is required. Not leaving all required information may slow the process of being added to the list, though, so make sure you at least leave your full name, date of birth and contact phone number!

Once vaccine doses have been allocated and made available, we will contact those on the eligibility list (from oldest to youngest as available) to register for a vaccine appointment.  

Yes, you can!  Should you need to cancel or reschedule a vaccine appointment, please do so as soon as the need arises. Cancelations and rescheduling will not change your status with respect to priority. We will work to place you with the next available dosage. If for any reason you cannot make an appointment, please cancel as soon as possible at (401) 643-0323 or (401) 477-2172 so we can provide another senior the appointment slot. We do not want to lose a dose that has a limited shelf life.

• If you have been scheduled for a vaccine, and another opportunity occurs for you to receive one sooner, let us know right away by calling (401) 643-0323 or (401) 477-2172 so we may assign the vaccine to another person.
• If you have NOT yet been scheduled for a vaccine, but are on our Eligibility List, please leave a message at (401) 643-0323 so we may take you off our list.

1. We strongly recommend you contact your medical provider for advice regarding inoculation with the vaccine.
2. Get on our Eligibility List while you decide. Remember, there is no obligation to get the vaccine when you are called, and the Registration Team will never pressure anyone into getting vaccinated. You have everything to gain and nothing to lose!
